Just got off the phone with OFAC. There isn’t a form for the special license application, we just have to submit a letter on company letter head that answers the 5 w’s (who, what, where, when, why) and all contact information for the company/individual. Once the letter is submitted we will receive a case number and then be waiting at least 2-3 months for processing before we get an answer.
