Good afternoon.  My name is Brian Jackson and I am assisting Teresa with your loan request.  I will be attending our meeting tomorrow afternoon.  I have attached our loan application as well as the 4506T form, and a business debt schedule for you to complete. 

 

The 4506T form is an authorization that gives our auditors permission to review your tax return.  We will need one for the business itself as well as one for each business owner.

 

The business debt schedule is a listing of any debt that THE BUSINESS has. 

 

If you have any questions, please give me a call.  We can assist you tomorrow with any parts you are unsure of.  Also, If you could please bring with you your most recent year’s tax return for the company and for the owners that would be helpful.  We may not need that for underwriting, but it may help with our meeting.  Finally, if you have an interim financial statement for the most recent month-end that would be great, too.
