The motion for Pro Hac Vice form is available on the courts website as a PDF fillable form.
Notices of Appeal fees (to include interlocutory appeal, cross appeal, or subsequent appeals) may be paid by credit card during the electronic filing of the notices.
Note: The fee for a Motion for Pro Hac Vice increased from $50.00 to $75.00 on November 1, 2011.
