RE: Sekneh Faraj File: FARA 0000316



I spoke with Sekneh and she told me that her employer sent them four times a letter stating what her duties are. And State Auto called her work and spoke with them.

Hello Mr. Faraj.

 

I am following up in regards to the lost wages for your client/our insured.  Per the note from Oakman Medical she is âpartially disabledâ with the following restrictions No lifting more that 20lbs.  In order to consider further lost wages, I need to find out what her exact job duties are as to why she is not able to return to work.  I will also need to have something in writing for her employer that they are not able to accommodate the above restriction: No lifting over 20lbs.  I have sent her employer a wage loss form as well as we will need that.

 

Also upon speaking to our nurse case manager she is not allowing her to attend any of her  medical appointments nor is she returning her calls and therefore I am wondering at why that would be.  The nurse  case manager was put on the  case to assist your client/our insured.

 

Therefore at this time I will not be issuing a wage loss check till all the above are accounted for.
