the new order adding on the full federal menu to your current plan. Please just
sign, date and certify the number of attorney’s in the firm (3) on the last
page. This needs to be hand written. Also, you do have the sdr linking
package currently in your plan if you would like to continue having this option
which allows you to link out from cases in your plan to cases outside your plan
do not initial under section 5.2. This option is an extra $23 per month.
