Are you registered in any other jurisdictions. Please list all of them. 3. I believe you are the sole owner of the company. If not, who else has an interest in it. 4. Please provide a narrative of the process of being removed from the VetBiz database. 5. Provide me a copy of the correspondence you received notifying of the removal decision. 6. Please provide dates of when you were notified, when you responded, and when they came back to you with denying and asking for additional information which the already have. 7. Cite, if you can, the law or regulation that recognizes your right to be listed in the database. 8. Estimate to the best of your abilities the amount of damages you believe you have suffered as a result of VA’s failure to act on your request. 9.
