The document I am looking for is an invoice from you covering the period of January 4, 2011 to January 18, 2011. On the CJA form which I filled out for you and you submitted, you claimed $6450 for compensation and $2391.70 for travel expenses including lodging, meals, parking etc…. You provided receipts for all your travel but have not provided me an invoice for the hours. As you may recall, the court approved your work at an hourly rate of $150. I need you to send me an invoice similar to the one you sent for the previous two submissions detailing the hours and when you did.
Once I receive that, we will put together the package one more time, send it to you for an original signature with a return envelope so you can send it back to me.
I attached an old invoice that you had sent previously as an example for you to use.
