Hi Marcy,Neal and Haytham,

I have attached the updated Group Short
and Long Term Disability plan. As a refresher Unum is the market leader
in Group Disability they provide an excellent program with a number of
employee support services.

The total monthly cost is  $624.67.
Please review the attached; once you confirm we will need to submit paperwork
which we will provide to Marcy.
